Contributions of artificial intelligence in the field of obesity

Resumen

La obesidad es un problema de salud a nivel mundial, que sigue creciendo y afecta tanto a adultos como a niños en todo el mundo. Este trastorno está relacionado con enfermedades graves como diabetes mellitus, aumento del riesgo cardiovascular y algunos tipos de cáncer. En este contexto, la inteligencia artificial se ha convertido en una herramienta útil para enfrentarse a los desafíos que produce la obesidad, ya que permite el análisis de datos de manera más profunda y detallada, descubriendo patrones y relaciones que antes no eran evidentes, y facilitando una medicina de precisión en estos pacientes. En el presente trabajo se realiza una revisión narrativa para analizar cómo se han aplicado las técnicas de Machine Learning y Deep Learning en la investigación sobre la obesidad con el fin de sintetizar las principales contribuciones que la inteligencia artificial ha ofrecido en este campo. La mayoría de los estudios, un total de 61, se centraron en la predicción de factores de riesgo de obesidad, mientras que hubo menos estudios sobre tratamiento y prevención, con nueve en cada área. Los estudios utilizaron dos enfoques principales del Machine Learning: técnicas clásicas y métodos de ensamblado. La técnica Support Vector Machines fue la más usada en clasificación y predicción, mientras que Regression (regresión logística) y Least Absolute Shrinkage and Selection Operator ayudaron a identificar las variables más importantes. Los métodos Random Forest y Extreme Gradient Boosting fueron los más utilizados en el análisis de datos complejos sobre obesi-dad. En Deep Learning, las Convolutional Neural Networks y las Recurrent Neural Networks fueron las más comunes, efectivas en el análisis de datos complejos como imágenes y secuencias temporales, mejorando el diagnóstico y la personalización de tratamientos. Además, la herramienta de inteligencia artificial explicable Shapley Additive Explanations fue la más usada para mejorar la interpretación de los modelos, clave en salud. La revisión identificó aportaciones en cuatro áreas principales: predicción de riesgos, personalización de tratamientos, diagnóstico temprano y análisis de factores biológicos relacionados con la obesidad. Los modelos permitieron identificar a niños en riesgo, estimar el índice de masa corporal (IMC) a partir de fotografías, anticipar la respuesta de los pacientes a distintos tratamientos, prever condiciones como la resistencia a la insulina, y estudiar determinantes biológicos como el microbioma. También ayudan a personalizar tratamientos, como la cirugía bariátrica, al anticipar la pérdida de peso potencial en cada paciente según sus características específicas. Sin duda, todas las herramientas que permite utilizar la inteligencia artificial con los pacientes mejorarán el conocimiento en los pacientes con obesidad, abriendo nuevas posibilidades diagnósticas y terapéuticas.

Abstract

Obesity is a growing global health problem affecting both adults and children worldwide. This disorder is linked to serious diseases such as diabetes mellitus, increased cardiovascular risk and some types of cancer. In this context, artificial intelligence has become a useful tool to face the challenges caused by obesity, as it allows the analysis of data in a deeper and more detailed way, discovering patterns and relationships that were not previously evident and facilitating precision medicine in these patients. This paper conducts a narrative review to analyze how Machine Learning and Deep Learning techniques have been applied in obesity research, in order to synthesize the main contributions that artificial intelligence has offered in this field. The majority of the studies, a total of 61, focused on predicting obesity risk factors, while there were fewer studies on treatment and prevention, with nine in each area. The studies used two main approaches to Machine Learning: classical techniques and ensemble methods. The Support Vector Machines technique was the most used in classification and prediction, while Logistic Regression and Least Absolute Shrinkage and Selection Operator helped identify the most important variables. Random Forest and Extreme Gradient Boosting methods were the most commonly used in the analysis of complex obesity data. In Deep Learning, Convolutional Neural Networks and Recurrent Neural Networks were the most common, effective in the analysis of complex data, such as images and temporal sequences, improving diagnosis and personalization of treatments. In addition, the Artificial Intelligence Tool Explainable: Shapley Additive Explanations was the most widely used tool to improve the interpretation of models, which is key in healthcare. The review identified contributions in four main areas: risk prediction, treatment personalization, early diagnosis and analysis of biological factors related to obesity. The models made it possible to identify children at risk, estimate body mass index from photos, anticipate patients’ response to different treatments, predict conditions such as insulin resistance, and study biological determinants such as the microbiome. They also help to personalize treatments, such as bariatric surgery, by anticipating potential weight loss in each patient according to their specific characteristics. Undoubtedly, all the tools that Artificial Intelligence allows us to use with our patients will improve our knowledge of patients with obesity, opening up new diagnostic and therapeutic possibilities.
